Proof of Income (if employed)
- Last 30 days of current pay stubs or a letter from your employer stating your earnings
Your ID (bring ONE of the following)
- Driver’s license
- Arizona State ID
- Birth certificate
- US passport
- Immigration or Naturalization certificate
- Medicaid card
Proof of US Citizenship and residency
- Social Security card or if you have the number memorized
- Birth certificate
- Residential address (not applicable if your stating homeless)
- Mailing address if different from above

- You may also apply for Nutrition Assistance and Cash Assistance.
- When submitting for Nutrition Assistance or Cash Assistance, you have 10 days to complete an in person interview at any local DES office. You may call the Help Desk for a possible phone interview 1-855-732-7587.
